chimatTo help support those working with children and young people with long-term health conditions such as asthma, diabetes and epilepsy, Public Health England has produced a Disease Management Information Toolkit (DIMIT).

Designed to increase the efficiency of services, the user-friendly format was originally developed by the Department of Health for the NHS to help identify which conditions contribute to emergency hospital admissions, bed stays and length of stay.

The user-friendly toolkit uses data from the Hospital Episode Statistics (HES), and Health and Social Care Information Centre, as well as a summary report of key data and information on trends over time and potential cost savings.
