In healthcare, the word transition is used to describe the process of preparing, planning and moving from children’s to adult services.
To support young people through transition, the University of Southampton's Childrens' Hospital has developed the Ready Steady Go transition programme.
In launching the programme a UoS spokesperson said: "We understand that moving away from a team of doctors and nurses that you have been with for many years can be scary but hopefully, by getting involved in the transition process, you will feel more confident and happier about the move."
Who is it for? You, if you or your child, is over 11 years old with a long-term medical condition.
What is it? A programme to help you get ready and feel confident to move to adult services around 18 years of age - at your own pace.
Why? Improves young people's health in the long-term and increases educational & job opportunities.
